HealthConnections

Getting the right healthcare is hard to do when you have a lot of different healthcare providers. You may have a primary care physician, specialists, and behavioral health providers. It can be hard to get the services you need, when you need them. It can be even harder to get all of your healthcare providers to talk with each other. Good healthcare is so important to your quality of life. That’s why HealthConnections offers tools to you and your healthcare providers to make your healthcare even better.

HealthConnections is an innovative program designed to help you improve and manage your physical and mental healthcare. You will work closely with our Nurse Navigator and Behavioral Navigator, who will work with your primary care doctor, any other healthcare providers you might have, and your insurance plans (Medicaid/Medicare) to coordinate care and ensure that you get the healthcare services you need, when you need them.

Additionally, our Navigators can:

  • Look at all the medications you are taking to make sure that they are safe to take together and are not causing problems and ensure that medications are refilled as necessary
  • Provide education about quitting smoking, healthy eating, exercise, fitness, and general wellness
  • Arrange travel to medical appointments
  • Help if hospital care is needed – Our Navigators will work with you and the hospital to make sure that you get better as soon as possible and receive the care and services needed after leaving the hospital.
